Initial removal of composite attachments with high speed burs, followed by fine removal with polishing burs and discs.
Bonded wire retainers tried in. Good fit.
Sandblasted wire and tooth.
Tooth washed. Etch 20secs, rinse, dry, bond . 5 secs LC.
Flowable composite placed. LC 20secs. Jig removed.
Bite checked. Essix retainers fitted, good fit. Advised wear and care.
Every night for 1st 6 months and 2-3 nights a week for life.
Whitening:
Risks and benefits discussed. Advised necks of teeth will appear darker and more limited in whitening ability.
Patient to use removable retainers as bleaching trays.
All instructions given and demonstrated. Written instructions given.

Add teardrop of gel in tray, ant teeth only. To seat and wipe off excess on gum with finger.
To wear for 6-10 hours a day.
To brush teeth like normal, brush teeth after use.
Brush and rinse trays with cold water, do not use bleach and hot water.
To use for 2-3 weeks for best result. To avoid staining foods during treatment.
Advised if sensitive teeth, to pause treatment and to place de-sensitising toothpaste in trays and seat.
Advised to store gels in cool dark area.
